These are the Enrollment Documents that are required to be completed for each student. Once completed, please return, along with the required documents at your scheduled Mandatory Parent Orientation Meeting.

Please note that your student(s) will not be fully enrolled until we receive all required documentation.

  Required only if the parent/guardian is living with someone else and there aren't any utilities or a lease in their name. Requires notarization
